    Haemorrhoids

    Overview

    There is a rapid and massive growth seen in the people suffering with lifestyle and metabolic disorder these days. One of such disease is haemorrhoids. Clinically, one out of every 5 individuals complains about having the disease. Haemorrhoid is a swelling observed in the anal and lower rectal veins. Increased pressure in the anorectal vein leads to haemorrhoids. It seems similar to varicose veins. It is commonly called “Piles” .

    Haemorrhoids in Ayurveda:

    In ayurveda, the disease is compared with a condition called “Arsha  roga”. Arsha are protrusion of swollen cell mass that tend to obstruct and irritate the person around the anal and the rectal region.

    Ayurvedic researches on Haemorrhoids:

    There are many researches taking place on haemorrhoids in various aspects. Most of them are based on the efficacy of various drugs  related to various bodily conditions associated with haemorrhoids.  According to a few researches, Rasaushadhi if used as per yukti of the physician, are more potent than other ayurvedic preparations.

    Common causes of haemorrhoids:

    1. Holding to a position (sitting or standing) of much longer time
    2. Dominance of heavy to digest food items in diet
    3. Untreated chronic diarrhea, constipation and IBS
    4. Low digestive power for a long period of time
    5. Controlling urge to defecate and urinate
    6. Low fibrous food content in diet
    7. Excess of masturbation
    8. Obesity
    9. Ageing
    10. Hereditary
    11. Anal intercourse

    Classification of Haemorrhoids:

    According to modern science:  Haemorrhoids can broadly be classified into 2 categories depending up on the location:

    1. External haemorrhoids: They are located below the dentate line, under the skin of the anus. They tend to appear more itchy, irritating and painful. The affected area appears swollen and often bleeds.
    2. Internal Haemorrhoids: They are located below the dentate line and are present under the rectal skin. They are often not felt are rarely cause discomfort. They are generally painless but can cause pain or irritation if stool is forced voluntarily to pass out.

    According to Ayurveda:

    1. Classification according to dominant dosha:
      Ayurveda has classified arsha roga on the basis of dominant dosha presented by the patient, which are as follows:
      • Vataj Arsha: This type of arsha is hard, brittle, and blackish in color. It is often accompanied with high intensity pain and massive constipation.
      • Pittaja Arsha: This type of arsha is exhibits burning sensation, redness and inflammation at the affected area. It is often accompanied with diarrhea and fever.
      • Arsha: It is slightly whitish in color and is soft and smooth to touch. It is often accompanied with poor digestion.
      • Sannipataj Arsha: It shows some symptoms of all the dosha.
      • Raktaja Arsha: it is often characterized by bleeding and many symptoms resemble to pittaja arsha roga.
      • Arsha: This is kind of arsha that exists in an individual by birth i.e. congenital.
    2. On the basis of tendency to bleed:
      • Shushka Arsha: It is referred as non-bleeding piles
      • Sraavi Arsha: It is referred as bleeding piles

    Common signs and symptoms of Haemorrhoids:

    1. Burning sensation in and around the anus after defecation
    2. Development of painful cell mass near anus
    3. Itching and irritation after defecation
    4. Pain during and after defecation
    5. Fresh bleeding while defecating
    6. Difficulty in sitting
    7. Leakage of stool

    Common signs and symptoms of Haemorrhoids:

    Patients suffering with Haemorrhoid/Arsha roga often experience painful defecation with history of chronic constipation or diarrhea.  They also experience itching, burning sensation and fresh blood oozing out. One might also experience the presence of skin tags around the anal area.

    Common signs and symptoms of Haemorrhoids:

    Patients suffering with Haemorrhoid/Arsha roga often experience painful defecation with history of chronic constipation or diarrhea.  They also experience itching, burning sensation and fresh blood oozing out. One might also experience the presence of skin tags around the anal area. 

    General complication of untreated haemorrhoids:

    Often people dare to neglect the presented symptoms and do get themselves diagnosed. Such undiagnosed and untreated conditions can often cause severely threatening complications like:

    • Strangulated haemorrhoid
    • Blood clots
    • Skin Tags
    • Infection
    • Prolapse
    • Anemia

    General Management of Haemorrhoids:

    Diet and lifestyle modification

    TO AVOID:

    1. consuming bulky and heavy to digest food items
    2. Sitting at one place for longer duration
    3. Do not stop the urge to defecate or urinate.
    4. Straining during bowel movement.
    5. Eating dry and spicy food items.

    TO DO:

    1. Eat food items rich in fiber e.g. nuts, whole grains, seeds.
    2. Massage with medicated oil or ghee around the anus
    3. Rush to washroom whenever required.
    4. Keep the affected area dry and clean
    5. Do optimum physical workout
    6. Maintain healthy weight.
    7. Drink plenty of water

    Effective Ayurvedic management of sinusitis:

    1. Medication: Minor haemorrhoid can be treated with medication as prescribed by a certified ayurvedic physician.
    2. Local Application of medicine (Kshara karma): Use of Kshara or Caustic, done by a certified and an experienced ayurvedic doctor on and around the skin tags and slits near, on and around the anal region.
    3. Surgical intervention (Shastra Karma): A therapeutic procedure popularly used in ayurveda to treat Arsha roga is Kshara sutra. In this procedure, a medicated threat is tied along the course of the haemorrhoid and as a result, within a span of 7-10 days, the haemorrhoid shrinks and detaches itself from the body.  Supporting oral medications are also used to aid the healing process.
    4. Cauterization (Agni karma): In this procedure, infrared heat is used to destroy the infected and the impacted cells. Merit of this procedure is, it ensures the non-recurrence of the disease.
    5. Bath: Sitz bath is not an ayurvedic method but it definitely helps in soothing pain caused due to haemorrhoids. One can perform it 10-20 minutes daily.

    FAQ's

    Are medications potent enough to treat haemorrhoids?

    Yes but it depends on the extent of progress of the haemorrhoid.  In severe cases, medications along with surgical interventions are required for proper treatment. 

    Can haemorrhoid occur even after the ayurvedic treatment?

    After taking consultation from a certified and experienced ayurvedic doctor and following all the medications and lifestyle modification as told, can help in avoiding the recurrence of the disease. But if failed to do so, it can cause the recurrence of the disease. 

    Is Kshara sutra an effective treatment?

    Yes. Kshara sutra is one of the most potent treatments of haemorrhoids. Its recovery is less painful and people can resume their work within a day or so. This surgical intervention is much easier than haemorrhoidectomy and takes less time to perform. 

    Can excessive spicy food cause heamrrhoid?

    Yes. Excessively spicy food items tend to vitiate pitta dosha in the body. The aggravated pitta dosha is highly potent in causing haemorrhoids. 

    Can Kshara sutra be done by MBBS doctor?

    No. Kshara sutra can only be done by certified Ayurveda practitioner.
