    Overview

    Gallstones are hardened deposits of digestive fluid that can form in the gallbladder. Gallstone disease is one of the most common problems affecting the digestive tract. The prevalence of gallstones is related to many factors, including age, Gender and ethnic background. Before adolescence, men and women seem to be equally affected, thereafter the disease is more prevalent in women. Women are three times more likely to develop Gallstone than men, and first-degree relatives of patients with gallstones have a twofold greater prevalence. According to Ayurveda, gallstones given the name Pitta ashmari, are a Kapha disorder associated with an underactive thyroid and slow metabolism.

    Mechanism of formation of GallStones?
    The formation of gallstones is a complex process that starts with bile, a fluid composed mostly of water, bile salts, and lecithin. Gallstones are usually caused by disturbances in the composition of the bile, a change in the ratio of cholesterol and bile salt may result in the formation of deposits. At the start, they may be in the form of fine gravel. But these fine particles constitute the nucleus for further deposits, ultimately leading to the formation of large stones. Irritation of the lining of the gallbladder due to inflammation may also lead to the formation of particles.

    What are the symptoms of stone in the gallbladder?

    1. Pain in the upper right side or upper middle of the abdomen In general gallstones are not painful. They remain in the gallbladder for a long time without causing any pain, indeed, you will not know they are there. Pain occurs when the gallbladder tries to push the stones out the gallbladder through the bile duct. The pain may be sharp, cramping or dull, spread to the back or below the right shoulder.

    How, your doctor can clinically diagnose gallstones?

    1. Murphy’s sign: During deep palpation of the right upper abdomen, the patient is asked to take deep breaths. The patient complains of severe pain during deep inspiration with inspiratory arrest, because the tender enlarged gall bladder comes down.
    2. Boas’s sign: If the patient is diagnosed with acute cholecystitis, this is an area of hyperaesthesia between the 9th and 11th rib posteriorly on the right side.

    Which investigation should be done for gallbladder stones?

    1. CBC: WBCs count is often elevated to 12000-15000/up
    2. Lipid profile: Mild elevation in serum bilirubin
    3. USG: Scan shows gallstone, sludge (mud) and thinking of the gallbladder wall
    4. Isotope scanning (HIDA, PIPIDA scanning)

    Management of Gallstones

    Prevention of gallstones and what to do when gallstones are present are two different lines of treatment. In general, gallstones are not painful. They may remain in the gallbladder for a long time without causing any pain, indeed you will not know they are there. Pain occurs when the gallbladder through the bile duct.

    Small gallstones can be removed with simple Ayurveda medicines and procedures, but large stones size more than 4mm, can be removed through surgery.
