    Overview

    Fissure is defined as any tear or cut to the anal region that can extend upto the anal canal. In ayurveda it is considered as ‘parikartika’ which means cutting pain at the anal region. It causes immense pain before the after the bowel discharge, burning sensation and something drops of fresh blood as well.

    Anal fissure is usually very small and upto 1cm only but due to it’s delicacy and sensitivity the pain prevaling from this area is very immense, severe and unbearable as well even a small cut can provide such a huge discomfort. Today, we will discuss all the aspects of fissure, its cause, symptoms, prevention and general management about it. Everything is going to be explained in detail in this article.

    Some general facts about anal fissure:

    Here are few general facts about anal fissure

    1. Damage to the lining of anus is very common case
    2. In some cases there is fissure for unknown reason
    3. Atleast 20 percent population effects from fissure somewhere in their life.
    4. Staying hydrated and preventing constipation can save us from fissure.

    General causes of fissure:

    Anal fissure can occur due to a majority of reasons:

    1. Constipation: it is considered to be the major cause for anal fissure, having hard and rough stools can easily tear the anal mucosa during passage of bowel, leading to anal Fissure.
    2. Diarrhoea: repeated diarrhoea leads to anal burn and soreness and it often causes fissure.
    3. Muscle spasm: any spasm to anal sphincter can increase the risk for anal fissure.
      What does spasm mean?
      Spasm means the automatic jerking of muscle movement, muscular spasm also interferes with the healing process.
    1. Pregnancy and labour: The pregnant women are at a high risk of developing fissure due to pressured bowel movement and during the labour process an expulsive pressure is given on perineum which may also tear the anal canal causing a fissure.
    2. Sexually Transmitted Disease: sexually transmitted diseases such as chlamydia, symphilis, and gonorrhoea are at high risk of developing the anal fissure.
    3. Anal Sex: impulsive penetration in anal sex, or sex toys may cause anal mucosal tear and cause fissure.
    4. Diseases of GIT: some git diseases such as ibs, ibd, ulcerative colitis, and Crohn’s diseases are likely at a high risk to develop anal fissure.

    Usually there are two muscular rings surrounding the anus and maintaining the control, one is the outer ring and the other is the inner ring

    Outer maintaining control and inner ring continuously maintains pressure. If the pressure goes beyond the limit inner ring does into muscular spasm causing pressure on blood vessels and resulting in fissure.

    Signs and symptoms associated with fissure:

    There could be many symptoms associated with anal fissure most common amongst them are follow:

    • Pain: pain is the major symptom of anal fissure and it is mainly associated with passing of stools. If the person pressurises himself to release stool and spent hours in washroom it gets worst
    • Blood: a bright red colored fresh blood is released from the anus and comes along with stools.
    • Itching: there is persistent (continuous) and intermittent itching around the anal region.
    • Dysuria: discomfort and pain during urination. Some patients may urinate frequently without any symptoms.

    Risk Factors

    Anal fissure can affect people of any age group or no gender diiference is there. It is the most common cause of bleeding from rectum in babies, childrens, womens and old aged.

    In most of the cases, anl fissures resolve without the need for medical treatment and surgery, superficial Creams, suppositories or both.

    An anal fissure that last less than 6 weeks is called an acute anal fisssure and a chronic fissure lasts for more than 6 weeks.

    Preventions

    1. Stool soft: keep the stool soft, eat more of the fibres in your diet to regulate the proper digestive process.
    2. Stop delaying washroom time: stop sitting for hours in washroom and pressuring yourself to pass stools it may lead anal tear due to pressure.
    3. Exercise: regular exercise can reduce the risk of developing constipation, as there would be low risk of constipation then the risk for developing anal fissure would be directly less.
    4. Wiping: do not wipe the anal region with dry or harsh cloth and paper, always use cotton cloth or soft moist paper for it because it may cut the anal region or make the fissure even worst.
    5. Napkin change: changing the diapers and nappies of the kids frequently prevents the risk to develop anal fissures in them

    Management

    1. Exercise: Daily exercise itself keeps the body healthy and prevents constipation and in turn prevents fissure as well. Exercise daily for at least 45mins to remain fit and healthy.
    2. Dietary regimen: constipation mainly leads to anal fissure in adults, increase the fibre intake in diet in order to maintain a good digestion and excretion of soft stools from anus. Include following food items in your diet for good digestion:
        • Vegetables and fruits.
        • Legumes -peas, dry beans, lentils etc
        • Soya and soya milk
        • Nuts and seeds.
        • Whole grain foods
        • Flax seeds, oats-bran, barley and psyllium husk should be used in meals.
    1. Sitz bath: having a tub bath filled with warm water in it for 15-20 mins is the best remedy for fissure at home .it reduces the inflammation and suppresses the swelling from anal region.
    2. Petroleum gel: apply petroleum jelly at anal region to reduce friction and mosturisen the anal opening in order to prevent the further carcks and tears.
